Please Note: 
A) Monthly fees are payable in advance and may be paid via debit order. Fees are due by the 3rd of each month, while             term fees are due by the end of the first week of the government school term. 
B) Classes  will be closed on ALL PUBLIC HOLIDAYS 
C) Termination of membership must be in writing with at least 1 MONTHS NOTICE and the current months fees will still        be due and NOT refundable.
D) Unfortunately no reduction in fees can be made for gymnasts on holiday or minor injury/illness

* Please supply any additional information on conditions that may require extra consideration by staff. It may be necessary to seek medical advice to confirm that participation in these activities will not have a negative impact on health. Medical information will be sought and where necessary any screening carried out prior to participation in the sport.

CONSENT :
As part of this initiative,  I understand that classes  are vigorous athletic activity. Due to the nature of the activity I also understand that the possibility of injury does exist.  I give my permission for coaches/instructors to authorize and or administer emergency First Aid.

I agree to release and hold harmless the Innov8 initiative as well as their partners, directors, employees and agents from liability for bodily injury, property damage or death arising from my or my minor child’s engagement in our services.
I hereby give this release and indemnification in exchange for the opportunity for me and my minor child to make use of the services.
This indemnity shall endure from this date  hereof until such time as I no longer engage in the services, or until such time as my minor child no longer engages in the services.

 In addition, ( bar our Muslim members )  I hereby give my consent to Innov8 to use any images or video recordings for training purposes within the initiative and for online promotional material.


I do hereby declare and certify that I have read this document and I fully understand its content. I am aware that this is an indemnity and release of liability and I sign it of my own free will.
